Important: The fee is non-refundable if you request a multiple-entry visa but are granted a single entry.

Important Reminders:
* The passport must not be used for at least one (1) month after submission. Borrowed passports are not allowed.
* The Application Form must be typewritten (not handwritten).
* Submit:
* Two (2) copies of the Authorization Letter, each with:
* A photocopy of a valid ID of the representative/applicant
* A photocopy of the passport’s bio page
* One (1) copy of the PSA Authorization Letter (per applicant)
* Consent Form for applicants 13 years old and below
* Do not staple any documents.
* Please remove issuance receipts of PSA certificates (not required).
